Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatric care experience to improve outcomes for long-term care residents. You will work with a team of providers and case managers to deliver coordinated, patient-centered care.

Our APRN/PA team will exercise independent judgment with physician supervision as needed, focusing on quality of life and reducing hospital visits. Sign-on bonuses and strong benefits accompany a growth-oriented environment.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and plan care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs and consultations.
• Monitor patient progress and adjust treatment plans.
• Collaborate with RN case managers as part of an integrated care team.
