Job summary

TruHealth seeks an advanced practice provider (NP/PA) with geriatrics focus to improve outcomes for long-term care residents through coordinated, evidence-based care with a multidisciplinary team.

You will work in nursing homes or assisted living settings, delivering personalized care and exercising independent judgement with physician supervision as appropriate. Excellent benefits and growth opportunities are offered.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a proactive plan of care.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order lab work, diagnostics and consultations.
• Monitor patients’ response to treatment and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
