Job summary

TruHealth seeks an advanced practice provider (NP or PA) with geriatrics experience to join our team serving seniors in long-term care settings. You will collaborate with physicians, nurses, and case managers to deliver coordinated, high-quality care.

Requires APRN/PA licensure, a master's degree or higher, and at least 3 years in geriatric clinical nursing or rehabilitation. Strong EHR skills and proficiency with Microsoft Office are expected.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assessing medical and mental health needs and creating a plan of care for residents.
• Ensuring compliance with local, state, and federal regulations related to clinical services.
• Prescribing medications and ordering lab work, imaging and consultations.
• Monitoring patient response to treatment and adjusting plans as needed.
• Collaborating with RN case managers as part of an integrated care team.
