Job summary

TruHealth is seeking an advanced practice provider (APRN/NP or PA) with geriatrics focus to join our care team in long‑term care settings. You will deliver proactive primary care, collaborate with RNs, case managers, and supervising physicians, and participate in implementing our national model of care.

Qualifications

• APRN or PA license required
• Degree from an accredited APRN or PA program
• 3 years’ experience in clinical nursing or rehab in geriatric populations
• Electronic Health Records experience
• Working knowledge of Microsoft applications, including Word, Outlook and Excel

Responsibilities

• Assess patients’ medical and mental health needs and plan proactive care
• Ensure compliance with local, state, and federal regulations related to clinical services
• Prescribe medications and order labs, diagnostics and consultations
• Monitor patient progress and adjust treatment plans as needed
• Collaborate with RN case managers as part of an integrated care team
