Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics experience to improve outcomes for long-term care residents. You will work with a team of clinicians and case managers to deliver personalized, coordinated care in nursing home or assisted living settings.

You will exercise independent judgment, treat acute and chronic conditions, and collaborate with facility partners and health plan leadership to implement TruHealth’s national model of care while complying with

Qualifications

• FNP, AGNP, AHACNP or PA license required
• Degree from an accredited APRN or PA program
• 3 years’ experience in clinical nursing or rehab in geriatric populations
• Electronic Health Records experience
• Working knowledge of Microsoft applications, including Word, Outlook and Excel

Responsibilities

• Assess patients’ medical and mental health needs and plan proactive care
• Ensure compliance with local, state, and federal agencies
• Prescribe medications and order lab work, diagnostics and consultations
• Monitor patient adherence and adjust treatment plans
• Collaborate with RN case managers as part of an integrated care team
