Job summary

TruHealth seeks an advanced practice provider (NP/PA) with geriatrics focus to enhance outcomes for long-term care residents. You will work with a team of providers and case managers to deliver coordinated, high-quality care in nursing homes and assisted living communities.

You will operate with independent judgement, manage acute and chronic conditions, and collaborate with facility partners to implement TruHealth's national model within regulatory guidelines.

Qualifications

• Master's degree or higher and APRN/PA program accreditation.
• Experience in clinical nursing or rehab, geriatrics preferred.
• EHR experience and MS Office proficiency required.

Responsibilities

• Assess medical and mental health needs and plan proactive care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications, order labs and consultations.
• Monitor treatment response and adjust plans accordingly.
• Collaborate with RN case managers in integrated care teams.
