Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatric primary care experience. You will work primarily in long-term care settings, delivering personalized, coordinated health care that improves residents' quality of life and reduces unnecessary hospital visits.

In this role you will exercise independent judgment, consult with a supervising physician as needed, and collaborate with facility partners and health plan leadership to implement TruHealth's national care model in

Qualifications

• Masters degree or higher in nursing or related field.
• APRN/PA licensure required (FNP/AGNP/AHACNP or PA).
• 3 years experience in geriatric clinical nursing or rehab.
• Proficient with Electronic Health Records.
• Strong MS Office skills (Word, Outlook, Excel).

Responsibilities

• Assess patients' medical and mental health needs and develop proactive care plans.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor and adjust treatment plans based on patient response.
• Collaborate with RN case managers as part of an integrated care team.
