Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA), with geriatrics/primary care experience to improve outcomes for long-term care residents. You will work primarily in nursing homes or assisted living communities enrolled in Medicare Advantage plans and collaborate with a team of providers and case managers.

You will exercise independent judgement to treat acute and chronic conditions while coordinating with supervising physicians

Qualifications

• Master's degree and APRN/PA license required.
• Minimum 3 years' geriatric clinical experience.
• Electronic Health Records experience.
• Proficient with MS Office.

Responsibilities

• Assess patients' medical and mental health needs and create a plan of care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs and consultations.
• Monitor treatment and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
