Job summary

AlignMed Partners is seeking an Advanced Practice Provider (NP/PA) for a flexible 20–40 hour schedule across three NH centers: Laconia, Concord, and Franklin. The role requires 1+ year of provider experience and supports on-site collaboration with leadership and medical staff.

The position offers a base annual salary of $120,640–$133,120 plus a no-cap incentive program, and a comprehensive benefits package, with relocation options for eligible candidates.

Qualifications

• Graduate from an accredited nursing program.
• Licensed registered nurse (RN).
• Certified Nurse Practitioner or Physician Assistant (PA) with required certifications and licensure.
• DEA license for each practicing state and PA licensure where applicable.

Responsibilities

• Provide primary and urgent care to nursing center residents, in collaboration with other providers.
• Collaborate with Center Leadership to meet AlignMed Partners' mission and goals.
• Report to Director of Clinical Practice/Area Clinical Practice Manager and work with Medical Director.
