AlignMed Partners in Franconia, NH, is seeking a Part-time Advanced Practice Provider (NP or PA) for Lafayette Center. This role supports primary and urgent care for nursing center residents, 2-3 days per week, 20-30 hours.

New grads are welcome. Base salary is $56–$64 per hour with an incentive productivity bonus, and the team collaborates closely with leadership to deliver high-quality care.

Qualifications

• Graduate from an accredited School of Nursing program, licensed as RN and certified as Nurse Practitioner with a Master's in Nursing and Older Adult/Family NP focus.
• PA must be graduate from ARC-PA accredited program and pass PANCE; state licensure and NCCPA certification required.
• Minimum two years experience with adult or geriatric population preferred.
• APRN with unrestricted license and DEA for each state; PA with unrestricted license and DEA.

Responsibilities

• Provide primary and urgent care to nursing center residents on-site with other providers.
• Collaborate with Center Leadership and Medical Director to meet center goals and AlignMed mission.
• Support clinical documentation and patient care delivery with the team.
