Job summary

AlignMed Partners is seeking a qualified Advanced Practice Provider (NP or PA) to deliver primary and urgent care to residents across three NH centers: Laconia, Concord, and Franklin. The role emphasizes collaboration with center leadership and medical directors, and requires a minimum of 1 year of provider experience.

Qualifications

Graduate from an accredited School of Nursing with RN licensure and NP credential (MSN) or PA with ARC-PA program and PA licensure.
NPs must have Older Adult and/or Family NP concentration; PAs must have PANCE and NCCPA certification.

Responsibilities

Provide primary and urgent care to nursing center residents.
Collaborate on-site with Center Leadership and Medical Director for patient care.
Support mission of improving lives through high-quality health care and compassion.
