Job summary

Optimal Care, Inc. is seeking a Nurse Practitioner or Physician Assistant to deliver primary and consultative geriatric care, often in patients' homes or care settings, with autonomy to focus on meaningful patient outcomes.

You will conduct comprehensive evaluations, develop individualized treatment plans, prescribe and monitor medications, and collaborate with physicians, caregivers, and a multidisciplinary team to prevent hospitalizations and improve quality of life.

Qualifications

• Board-certified NP or PA with current state license.
• Master’s or doctoral degree in nursing or physician assistant practice.
• Reliable transportation and valid driver’s license.
• Minimum 2 years clinical post-acute experience; geriatric care preferred.

Responsibilities

• Perform comprehensive medical evaluations and manage treatment plans.
• Prescribe and monitor medications; coordinate with physicians and caregivers.
• Deliver high-quality, patient-centered care in home or care settings.
• Act as a clinical decision-maker and help families understand diagnoses.
