Job summary

ChenMed is seeking a Nurse Practitioner to join our clinical operations team. The NP will provide direct patient care across ChenMed centers, SNFs, and home settings, conducting geriatrics assessments, exams, diagnoses, and care plans in collaboration with physicians and care teams.

The role requires state NP licensure, DEA registration as needed, BLS, and at least 2 years of family/internal/geriatric practice with autonomy. Travel up to 50-75% may be needed.

Qualifications

• ARNP or similar advanced degree in Nursing is required.
• Current Nurse Practitioner Certification in the state of practice is required.
• Board certification by AANP or ANCC is preferred but may be required for certain states.
• Current DEA number for Schedule II-V may be required depending on state.
• Minimum 2 years of clinical NP experience in Family/Internal/Geriatrics with autonomy.
• BLS certification within first 90 days of employment.

Responsibilities

• Independently assesses acute and non-acute clinical problems and documents histories and plans.
• Plans patient care based on population knowledge, and serves as patient advocate.
• Writes orders for admissions, transfers, discharges; orders labs and treatments.
• Refers patients for specialist consultation when indicated.
• Documents progress notes and summaries; may perform certain procedures with competency.
• Collaborates with multidisciplinary team to optimize patient care and outcomes.
