Job summary

ChenMed in Illinois is seeking a skilled Nurse Practitioner to join our clinical operations team, delivering direct patient care across ChenMed centers, SNFs, and home settings. You will conduct assessments, develop care plans, perform histories and exams, and collaborate with physicians to ensure high-quality, coordinated care for our senior patients.

Documentation in ChenMed systems, participation in rounds, and adherence to regulatory standards are expected, with focus on patient education

Qualifications

• ARNP or similar advanced degree in Nursing (required).
• Current Nurse Practitioner Certification in the State of practice (required).
• Board certification by AANP or ANCC is preferred.
• Illinois license with full practice authority.
• BLS certification within first 90 days of employment.

Responsibilities

• Independently assesses acute and non-acute clinical problems and documents findings.
• Develops patient management plans considering patient population and cultural background.
• Orders and interprets labs and diagnostic studies; manages medications and treatments.
• Refers patients for specialty consultations as needed and documents progress.
• Participates in rounds, communicates management strategies, and collaborates with the care team.
