Job summary

ChenMed is seeking a Nurse Practitioner to join our clinical operations team providing high-quality care across centers, SNFs, and home settings. You will conduct comprehensive assessments, plan and implement care, order diagnostics, and collaborate with physicians to optimize patient outcomes.

The role requires ARNP/NP credentials, state licensure, and strong patient communication. Travel up to 50-75% is possible, with a pay range that reflects experience and location.

Qualifications

• ARNP or similar advanced degree required.
• Current Nurse Practitioner Certification in the State of practice required.
• Board certification by AANP or ANCC preferred.
• Current DEA number from the DEA may be required depending on State.
• Minimum 2 years of clinical NP experience in Family Medicine/Internal Medicine/Geriatrics.

Responsibilities

• Assess acute and non-acute clinical problems; perform exams and histories; develop a patient management plan.
• Plan patient care considering patient population, culture, and support systems; advocate for the patient.
• Write orders for admissions, transfers, discharges; interpret labs and diagnostics; refer to specialists as needed.
• Document progress notes and summaries; participate in rounds and conferences; communicate management strategies with the care team.
• Perform invasive procedures when competency is documented; collaborate with physicians and multidisciplinary teams; initiate life-saving actions when required.
