Job summary

ChenMed is seeking a Nurse Practitioner to join our clinical operations team. You will provide direct patient care across ChenMed centers, SNFs, and home settings, perform assessments, and develop nursing plans of care in collaboration with physicians and care teams.

The role requires independent practice, adherence to ChenMed standards, and engagement in rounds, documentation, and disease management to improve patient outcomes. Travel up to 50–75% may be required depending on assignment.

Qualifications

• ARNP or similar advanced degree is required.
• Current Nurse Practitioner Certification in the state of practice is required.
• Board certification by AANP or ANCC is preferred but may be required for certain States.
• DEA registration may be required based on state of practice.
• Minimum 2 years of clinical experience as a Nurse Practitioner in Family/Medicine or Geriatrics.
• BLS certification from AHA or Red Cross required within first 90 days.

Responsibilities

• Independently assesses acute and non-acute clinical problems, documents histories, and develops a patient management plan.
• Plans patient care based on population-specific knowledge and protocol, advocating for patients.
• Writes orders for admissions, transfers, and discharges.
• Orders and interprets labs and diagnostic studies; prescribes medications.
• Refers patients for specialist consultation when indicated and coordinates care.
• Documents progress notes and summaries in detail.
• May perform invasive procedures with demonstrated competency.
• Participates in rounds and communicates management strategies to the care team.
• Recognizes urgent situations and initiates physician involvement when needed.
• Uses advanced communication to improve patient care and processes.
