Job summary

TruHealth in Iowa is seeking an advanced practice provider (NP or PA) with geriatrics experience to improve care for long-term residents. You will work with a team to deliver personalized, coordinated healthcare in senior living facilities.

The role emphasizes independent clinical judgment, collaboration with case managers, and adherence to regulations while advancing outcomes and quality of life for patients.

Qualifications

• Masters degree or higher in nursing or physician assistant studies.
• Licensed APRN (FNP/AGNP/AHACNP) or PA with geriatrics focus.
• 3 years’ experience in geriatric clinical nursing or rehab.

Responsibilities

• Assess patients and develop proactive care plans for geriatrics.
• Coordinate care with RN case managers and physicians.
• Prescribe medications and order labs as appropriate.
• Ensure compliance with local, state, and federal regulations.
• Document care plans and monitor treatment outcomes.
