Job summary

TruHealth in Iowa is seeking an advanced practice provider (NP/PA) with geriatric focus to improve outcomes for long-term care residents. You will collaborate with a multidisciplinary team, including case managers, to deliver coordinated, patient-centered care in nursing homes or assisted living communities.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Masters or better in nursing or related field.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.

Responsibilities

• Assess patients’ medical and mental health needs and plan proactive care.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications, order labs and diagnostics, and coordinate care with the team.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
