Job summary

TruHealth in Iowa seeks an advanced practice provider (NP or PA) with geriatrics focus to improve outcomes for long-term care residents. You will collaborate with a multidisciplinary team to deliver coordinated care and enhance quality of life for patients.

You will work primarily with nursing homes and assisted living facilities enrolled in Medicare Advantage plans, exercising independent judgement with physician oversight and ensuring regulatory compliance.

Qualifications

• APRN or PA license required.
• Master's degree or higher in nursing or related field.
• 3 years' clinical nursing or rehab experience in geriatric populations.
• Electronic Health Records experience.

Responsibilities

• Assess patients and develop care plans for aging populations.
• Prescribe medications and order labs and imaging as needed.
• Coordinate with care team including RNs and case managers.
• Ensure regulatory compliance of clinical services.

Education

Masters or better

Degree from accredited APRN/PA program
