Job summary

Scan Health Plan in California seeks an Advanced Practice Clinician (NP/PA) to provide and coordinate primary care for SCAN members in their homes or senior living settings.

You will perform exams, vaccinations, tests, and chronic disease management, while coordinating with case management and specialists to ensure timely care. Remote/field-based with significant travel and after-hours support.

Qualifications

• Master’s degree and applicable licensure in California.
• California NP license or PA-C in good standing; Wound Care and Palliative Care certificates a plus.
• Knowledge of CMS guidelines and Medicare Advantage Managed Care.

Responsibilities

• Provide and coordinate primary care to SCAN members in their place of residence.
• Conduct exams, vaccines, diagnostics, and care planning.
• Collaborate with case management and other providers to ensure timely access to services.
• Coordinate with specialists and community resources for continuity of care.
• Document visits promptly and maintain HIPAA compliance.
• Support after-hours telehealth and on-call coverage.
