Job summary

SCAN Health Plan seeks an Advanced Practice Clinician to provide and coordinate primary care for seniors in their residences, typically in senior living communities or skilled nursing facilities. You will conduct exams, perform triage, order tests, and coordinate with case management to ensure timely, appropriate care.

The role emphasizes chronic disease management, goals-of-care conversations, and collaboration with Primary Care Physicians and specialists to promote prevention and improve

Qualifications

• Current California Nurse Practitioner license or California Physician Assistant Certification in good standing.
• Wound Care and Palliative Care certificates are a plus.
• Experience with chronic disease management and geriatric care is preferred.

Responsibilities

• Provide and coordinate primary care to SCAN members in their place of residence.
• Conduct physical/wellness exams and assessments; perform triage to avoid unnecessary hospital visits.
• Order and interpret diagnostics; coordinate with care teams to close care gaps and manage chronic diseases.
