Job summary

SCAN Group is seeking an Advanced Practice Clinician to provide and coordinate primary care for SCAN members in their residences, primarily in senior living facilities. You will perform exams, manage chronic disease, and lead care plans with a focus on keeping seniors healthy and independent.

The role requires an NP/PA with geriatrics experience, Arizona licensure, and the ability to travel frequently within the assigned area.

Qualifications

• Active Arizona NP license or PA-C certification in good standing.
• Master’s degree in nursing or PA program; geriatric care preferred.
• Experience with CMS guidelines and Medicare Advantage.
• Willingness to travel 75%+ and accommodate flexible scheduling.

Responsibilities

• Provide care to an assigned panel of members in their residence (senior living communities or SNFs).
• Conduct physical/wellness exams and urgent visits; perform vaccinations and tests.
• Collaborate with case management for timely access to services.
• Document visits in EMR and coordinate with care teams.
• Engage with primary care physicians and specialists for comprehensive care.
• Support care gap closure and optimize clinical workflows.
