Job summary

SCAN Health Plan is seeking an Advanced Practice Clinician to provide and coordinate primary care for members in their residences, including senior living communities and SNFs. The role involves exams, vaccines, chronic disease management, and telephonic triage to prevent hospitalizations.

Ideal candidates hold a California NP license or PA-C, with Wound Care or Palliative Care credentials being a plus, and flexible travel of 75%+.

Qualifications

• CA NP license or CA PA-C in good standing.
• Wound Care and Palliative Care certificates are a plus.
• Experience with complex chronic diseases (dementia, diabetes, heart failure, COPD).
• Willingness to travel 75%+ in the assigned area.

Responsibilities

• Provide and coordinate primary care to SCAN members in their residences.
• Conduct physical exams, vaccines, and chronic disease management.
• Coordinate with case management and care teams for timely services.
• Document visits per protocol and maintain HIPAA compliance.
• Participate in after-hours on-call and virtual visits as needed.
