Job summary

SCAN Health Plan seeks an Advanced Practice Clinician to provide and coordinate primary care for members in their residences, often in senior living communities or skilled nursing facilities. You will perform exams, vaccines, and triage to prevent hospital visits, while building care plans with families and staff.

You will collaborate with a care team, coordinate with physicians, and participate in on-call coverage after hours and weekends.

Qualifications

• Master’s degree in Nurse Practitioner or equivalent PA program.
• Education in Adult/Geriatric care preferred.
• Knowledge of CMS guidelines and Medicare Advantage Managed Care.

Responsibilities

• Provide and coordinate primary care to SCAN members in their place of residence.
• Conduct physical/wellness exams and vaccines; triage to ensure timely care.
• Collaborate with case management for timely access to services.
