Job summary

SCAN is seeking an Advanced Practice Clinician to provide and coordinate primary care for seniors in their homes and community settings. The role involves exams, triage, vaccines, and chronic disease management, with travel across designated regions.

You will collaborate with the care team, engage with physicians, coordinate with specialists, and maintain documentation per protocol. This patient-facing position supports SCAN’s mission to keep seniors healthy and independent.

Qualifications

• Master’s degree in Nurse Practitioner or equivalent PA program.
• California NP license or CA PA-C in good standing.
• Wound Care Certificate and Palliative Care Certificate are a plus.
• Knowledge of CMS guidelines, ICD-10 coding, and Medicare Advantage.

Responsibilities

• Provide and coordinate primary care to SCAN members in their residence or community setting.
• Identify health care needs and develop a comprehensive plan of care with patient and family.
• Collaborate with case management to ensure timely access to services.
• Manage visitation schedule per practice protocols and member needs.
• Coordinate with specialists, hospitals, and community organizations for continuity of care.
