Job summary

Total Life Inc. is seeking a Nurse Practitioner to lead longevity visits at partner senior living communities in the Joliet-Palatine area. You will conduct comprehensive geriatric assessments, deprescribing, and cognitive screening, while coordinating with primary care providers.

The role is a contract position with on-site days on weekdays; you will receive training from our CMO and support for onboarding, with travel between communities as bookings grow.

Qualifications

• Active Illinois APRN license and RN license
• Current national NP board certification
• DEA registration and state practice authority as required in Illinois

Responsibilities

• Lead longevity visits and comprehensive geriatric assessments
• Coordinate with primary care providers and document to Medicare standards
• Travel to Joliet-Palatine area site days and participate in education talks
• Maintain accurate records and ensure compliance with regulatory requirements
• Represent Total Life professionally with residents and staff
