Total Life is seeking nurse practitioners to lead longevity days in Inland Empire senior living communities. You will deliver 45–60 minute visits based on a comprehensive geriatric assessment, review medications, and address mood, sleep, nutrition, mobility and cognitive health.

You’ll train under a geriatrician and travel between sites. The role requires CA NP certification, RN license, DEA and CURES registrations, and ability to work weekdays in person.

Qualifications

• Active, unrestricted California RN license and NP certification.
• Current NP Furnishing Number from California BRN.
• DEA registration for California and CURES enrollment.
• Experience with Medicare documentation and E/M coding.
• Clinical experience caring for adults 65+.

Responsabilités

• Lead longevity visits after geriatric assessment, including meds review and deprescribing.
• Identify residents for follow-up care and coordinate with primary providers.
• Document encounters to Medicare standards in our EHR.
• Present professionally to residents, families and staff at communities.
• Travel to assigned Inland Empire senior living communities.
