HEALTH AND HUMAN SERVICES COMMISSION is seeking a Community Care Case Worker to assess, authorize, and monitor long-term care services for clients across Region 10. Qualifications

• Knowledge of customer services principles and practices.

Responsibilities

• Interviews clients or resource persons and conducts home visits to assess service needs.
• Calculates income and resources to determine client financial eligibility.
• Develops and coordinates client services and payment plans.
• Authorizes services and ensures contract agencies meet client needs.
