Job summary

HEALTH AND HUMAN SERVICES COMMISSION is seeking a Community Care Case Worker to assess, authorize, and monitor long-term care services for clients across Region 10. You will interview clients, verify data, determine eligibility, and coordinate services with contract agencies to meet individual needs.

The role requires experience in interviewing clients, documenting information, and using automated systems for case management. Strong communication with diverse populations is essential for success.

Job Description

• This position will be assigned to
• Region
• 10 HHSC-CCSE Unit 22 comprised of
• approximately 11-14 Community
• Care Case Worker staffcovering
• all 6 counties in Region 10.
• Provides direct delivery and
• contract services to long term
• care services clients in assigned
• geographic area.
• Determines/re-determines need and
• eligibility for Community Care
• Services, authorizes and monitors
• Community Care Services provided
• by contract agencies. Work
• involves managing a caseload,
• interviewing elderly clients and
• clients with disabilities,
• documenting information,
• determining need for and amount
• of services, verifying case data,
• explaining program benefits and
• requirements, referring clients
• for appropriate services and
• assuring authorizations are
• registered on automated systems.
• Work includes operating a
• computer.

Essential Job Functions

• Attend work on a regular and

• operations. May review situations
• and provide temporary protective
• services to prevent or remedy
• abuse, neglect, or exploitation
• of elderly or disabled adults.
• Performs related work as
• assigned.

Registrations, Licensure

• Valid drivers license required.
