Job summary

Job Board is seeking a Community Worker-Senior Center/SHARP professional in Flushing, NY for on-site, full-time work. You will assess geriatric clients, coordinate services, and advocate for community resources while maintaining thorough records and reporting.

Responsibilities include intake, in-home assessments, crisis intervention and collaboration with healthcare and social services, with emphasis on respectful client interaction and compliance with regulatory standards.

Qualifications

• Bachelor's Degree required
• One year related experience and/or training
• Working knowledge of Microsoft Office Suite and other technology
• Excellent communication and listening skills
• Excellent customer service skills with a focus on treating clients with respect and dignity
• Able to multi-task and work independently with a great attention to detail

Responsibilities

• Provide assessment, service coordination and counseling to geriatric clients eligible for multiple services
• Provide case management services including intake, ongoing advocacy, in-home assessments and crisis intervention
• Prepare reports and maintain proper client files with documentation
• Coordinate client care with supervisors and nurses as needed
• Attend trainings, meetings and keep supervisor informed of activities and needs
• Adhere to mandatory trainings on topics including HIPAA, OSHA and regulatory compliance
