Aging and Long Term Care of Eastern Washington is seeking a Case Manager Assistant to support Case Managers by handling client information, performing home visits, and coordinating services. Qualifications

• One year of in-person human services experience (paid or volunteer).
• High school diploma or equivalent required.
• Must pass a criminal background check after offer.
• Must have and maintain a valid driver’s license.

Responsibilities

• Provide direct assistance to providers and clients by phone or in person.
• Assist walk-in Case Management clientele.
• Assist clients in obtaining equipment and supplies per Case Manager direction.
• Conduct home visits as directed by the Case Manager.
• Complete documentation of Service Episode Records (SER) in CARE.
• Authorize services using the ProviderOne system.
• Perform data management, editing and report generation in agency/governmental systems.
• Track and distribute documents and forms following program policy.
• Maintain Case Management forms inventory.
• Other duties as assigned.
