Job summary

Golden Years is seeking a HCB Waiver Case Manager to provide comprehensive case management for clients enrolled in the Home and Community-Based Waiver program. Responsibilities include assessing eligibility, creating individualized care plans, and coordinating services with providers and community resources to ensure safety and well-being.

The role requires strong communication, organization, and advocacy skills, as well as the ability to manage documentation and maintain compliance with program

Qualifications

• Bachelor's degree in health and human services or equivalent.
• At least 1 year of health/human services experience; aging or disabilities focus preferred.
• Registered Nurse with a master's in health/human services (RN with MS preferred).

Responsibilities

• Assess and plan care to determine eligibility for the HCB Waiver program.
• Develop and maintain individualized care plans and monitor eligibility.
• Coordinate service delivery and assist clients in accessing community resources.
