Job summary

Element Care PACE seeks a Physician to provide comprehensive geriatric primary care within the PACE Interdisciplinary Team, owning participant outcomes and center results while supporting independence and safe community living.

Responsibilities include managing complex illness, performing exams, optimizing medications, telehealth, home visits, leadership participation on Center Leadership Team, and advancing value-based care across the program.

Qualifications

• Completed medical residency required; Internal Medicine, Family Medicine or Emergency Medicine preferred.
• Board certification or eligible status for recent graduates required.
• Experience in geriatric care and population health is desirable.

Responsibilities

• Provides geriatric primary care for complex illness, frailty, cognition, function and prevention with team-developed care plans.
• Performs clinics, home visits and required nursing-home visits; uses telehealth and coordinates transitions.
• Leads medication optimization and deprescribing in collaboration with the care team.
• Participates in Center Leadership Team and uses population health principles to reduce gaps and improve quality.
• Coordinates with behavioral health and supports caregivers and diverse participant needs.
