Job summary

Center for Elders’ Independence (CEI) in California seeks a Primary Care Physician to oversee medical care for participants in the PACE program. You will lead an interdisciplinary team, guide medical decisions, and coordinate care with families and hospital services.

The ideal candidate has an MD, a California medical license, and experience with frail elders. Board certification in Internal Medicine or Family Practice is preferred, with geriatrics experience a plus.

Qualifications

• MD degree and California medical license to practice medicine.
• Minimum of 1 year of experience with frail/elderly population.
• Board certification in Internal Medicine or Family Practice preferred; geriatrics subspecialty preferred.
• BLS certification required; commitment to PACE interdisciplinary team.
• Strong communication and leadership skills; ability to collaborate across a multi-disciplinary team.

Responsibilities

• Provide comprehensive medical care for program participants under the PACE model.
• Lead and educate the interdisciplinary team on medical issues.
• Review and sign care plans every 6 months.
• Supervise CEI nurse practitioners and physician assistants as needed.
• Coordinate care with hospitalists, specialists, and family networks; ensure appropriate follow-up.
• Contribute to quality assurance and compliance with CEI policies.
