Qualifications

Responsibilities

• Provides geriatric primary care for complex conditions and init/exam care plans.
• Performs home visits, clinic care and nursing facility rounds as needed.
• Integrates palliative care and coordinates with behavioral health.
• Participates in Center Leadership Team and improves workflows.
• Ensures safe care transitions and effective use of resources.
