Job summary

Habitat Health seeks a compassionate Nursing Assistant to support daily living activities, assist with mobility and personal care, and help maintain participants’ well-being in our PACE setting. The role emphasizes safety, data entry, and adherence to care plans in a fast-paced environment.

Applicants should have an accredited vocational nursing assistant program and a current Food Handler's Certification or obtain it within 30 days.

Qualifications

• Completion of accredited vocational nursing assistant course or comparable participant care experience.
• Food Handler's Certification required; must obtain within 30 days of start date.

Responsibilities

• Assist participants with activities of daily living (ADL) and personal hygiene.
• Prepare and plate meals according to dietary plans and preferences.
• Collect data in EMR and ensure accuracy and timeliness.
• Provide homemaking services and light housekeeping as outlined in plan of care.
• Accompany participants to appointments and conduct reminder calls.
• Monitor safety, infection control, and adherence to PPE.
