Job summary

Habitat Health in San Diego County is seeking a compassionate Nursing Assistant to support daily living activities and in-home assistance for participants in our PACE program. You will provide meals, mobility help, and safety-focused care, working closely with a multidisciplinary team.

The role requires completion of a certified nursing assistant program and CPR/BLS, with a baseline of one year caring for the frail or elderly. The position offers competitive hourly pay and comprehensive benefits.

Qualifications

• Completion of accredited vocational nursing assistant course, hospital, or comparable participant care experience.
• Knowledge of PACE program.
• Ability to work without close supervision and exercise independent judgment.
• Knowledge of physical, mental, and social needs of frail older adults.
• Ability to manage changing priorities per participant needs.
• Effective interpersonal skills to work with participants, caregivers, and staff.
• Ability to work with diverse ethnic and cultural backgrounds.
• Requires physical strength to perform essential functions.
• Minimum of one year experience working with the frail or elderly.
• Proof of medical clearance for communicable diseases, including TB test.
• Proof of all immunizations up to date.
• Proof of current CPR/BLS certification or requirement to obtain within 30 days.
• Proof of valid CA driver’s license, transportation, and insurance as required by state law.

Responsibilities

• Contribute to a center experience that participants want to spend time in, with a caring team culture.
• Assist participants with activities of daily living (ADL) including feeding and ambulation.
• Prepare and plate food for meal service per dietary cards.
• Maintain participant comfort and privacy; keep supplies within reach.
• Follow procedures and maintain clinical competencies for assigned tasks.
• Record data and participant care information accurately.
• Provide safety guidance related to care plans and infection control.
