Qualifications

• High School diploma or GED required.
• Minimum of one year as a CHMG Patient Care Coordinator preferred.
• Knowledge of scheduling, EMR, referrals, authorizations and payor guidelines preferred.

Responsibilities

• Greet patients in person and on the phone in a cordial, professional manner.
• Facilitate patient care between specialty, primary and ancillary services.
• Spend at least 70% of time scheduling clinic and diagnostic testing/procedure appointments.
• Obtain prior-authorization for diagnostic testing/procedures with payers.
• Schedule pre and post-diagnostic labs and appointments as required.
• Complete diagnostic testing/procedure questionnaires.
• Educate patients on diagnostic study/procedure preparation.
• Support communication among clinicians by ensuring notes and information are available.
• Communicate with outside offices, facilities and hospital departments to facilitate care.
• Research incomplete or incorrect orders.
• Prioritize tasks while serving patients at the window.
• May support other administrative functions as needed.
