Qualifications

• Active, unrestricted medical license(s) in coverage area(s).
• Ability to obtain and maintain multi-state licenses as required.
• Experience or capability to perform in-home health evaluations.

Responsibilities

• Provide in-home health evaluations across a defined coverage area.
• Travel as needed, up to 100%, including potential overnight stays.
• Utilize iPad-based workflows to conduct visits and document findings.
• Collaborate with health plans to bridge gaps in care and coordinate follow-ups.
