Job summary

Qualifications

• High school diploma or equivalent is required.
• Bachelor's degree is preferred for related field.
• 3+ years of professional work experience with a high school diploma.
• 1+ year of professional work experience with a Bachelor's degree.

Responsibilities

• Outreach and recruitment of participants for Family Resource Center activities.
• Establish and maintain a professional relationship with families with regular contact.
• Administer intake, assessments, and screening tools to clients and family members.
• Use intake/assessment results to set goals, objectives, and activities with parents.
• Provide intensive case management services to families.
• Coordinate service planning, referrals, and transportation for families on caseload.
• Conduct systematic follow-ups for all families.
