Job summary

BCFS Health & Human Services is seeking a Resource Navigator to ensure quality services for families while adhering to program standards. The role involves outreach, intake, assessments, and coordinating services with a multidisciplinary team.

Ideal candidates will have a high school diploma (or equivalent) and 3+ years of related experience; a Bachelor's degree is preferred, with at least 1 year of related experience. Texas driver’s license and background checks are required.

Qualifications

• Required: High school diploma or equivalent.
• Preferred: Bachelor's degree in a related field.
• Experience: With a high school diploma, at least 3 years of professional work experience in a related field.
• Preferred: With a Bachelor’s degree, at least 1 year of professional work experience in a related field.

Responsibilities

• Outreach and recruitment of participants for Family Resource Center activities.
• Establish and maintain a professional relationship with families with regular contact.
• Administer intake, assessments, and screening tools to clients and family members.
• Use intake/assessment results to set goals and activities with parents.
• Provide intensive case management services to families.
• Coordinate services, resources, referrals, basic needs support, transportation, and other support.
• Conduct systematic follow-ups for all families.
