Job summary

BCFS in Texas is seeking a Resource Navigator to provide high-quality services to families while ensuring compliance with program standards. You will build trusting relationships, conduct intakes, set goals with parents, and deliver intensive case management within a multidisciplinary team.

Responsibilities include coordinating resources, documenting activities in Salesforce and PEIRS, and performing follow-ups.

Qualifications

• High school diploma or equivalent required.
• Bachelor’s degree preferred in a related field.
• 3+ years of professional work experience with a high school diploma; 1+ year with a Bachelor's degree preferred.

Responsibilities

• Outreach and recruitment of participants for Family Resource Center activities.
• Establish and maintain a trusting and professional relationship with families through regular contact.
• Administer intake, assessments, and screening tools to clients and family members.
• Use intake results to establish and implement goals, objectives, and activities with parents.
• Provide intensive case management services to families.
• Provide service planning, coordination, referrals, basic needs support, transportation assistance, and other services to families on caseload.
• Conduct systematic follow-ups for all families.
• Collaborate with Lead Resource Navigator and other Navigators on family activities and caregiver support groups.
• Maintain documentation in Salesforce and PEIRS within specified timeframes.
• Attend required trainings and contribute to monthly updates and education information.
