Job summary

WelbeHealth is seeking a Home Care Assistant to provide in-home personal care and support services for participants in their homes across assigned schedules. You will perform ADLs, assist with instrumental tasks, and document vital signs while working as part of an IDT and transportation to medical appointments.

This role offers a stable full-time schedule with base pay plus bonus, travel time and mileage reimbursement, and comprehensive benefits including medical insurance and a 401K match.

Qualifications

• High school diploma or equivalent required.
• BLS/First Aid certification required.
• CNA or HHA certification preferred.
• Bilingual ability preferred; translation support may be needed.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living (ADLs) in participants' homes.
• Assist with Instrumental ADLs such as laundry, light housekeeping, shopping, and meal prep.
• Measure and document participants’ vital signs.
• Act as a medical escort on vehicles to medical appointments; assist transfers.
• Support Rehabilitation team with home exercise programs per care plan.
• Provide translation services as needed.
