Job summary

WelbeHealth Home Care Assistants provide essential in-home care to participants, delivering personal care and assisting with daily living activities within their homes, and contributing to care planning as part of the interdisciplinary team. The role offers a stable full-time schedule with travel between participants and reimbursement for travel time and mileage.

Qualifications

• High school diploma or equivalent required.
• Current BLS/First Aid certification required.
• CNA or HHA certification preferred to support home care duties.

Responsibilities

• Provide hands-on personal care and assistance with ADLs in participants' homes (bathing, dressing, grooming, toileting, safe transfers).
• Assist with Instrumental ADLs (laundry, light housekeeping, shopping, meal prep).
• Measure and document vital signs as instructed.
• Act as medical escort to appointments and assist with safe transfers from home.
• Support Rehabilitation team by carrying out home exercise programs per care plan after training.
• Provide translation services as needed.
