Zusammenfassung

WelbeHealth is seeking Home Care Assistants to deliver hands-on personal care and support to participants in their homes, coordinating with the IDT to execute care plans. You will travel between participants, receive reimbursements for travel, and join a team that values stability, comprehensive benefits, and a competitive compensation package.

Requirements include a high school diploma, BLS/First Aid, and optional CNA/HHA certification.

Qualifikationen

• High school diploma or equivalency.
• Current Basic Life Support (BLS)/First Aid Certification required.
• Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred.
• Bilingual preferred.
• Must be registered with Home Care Registry prior to start date.
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

Aufgaben

• Provide hands‑on personal care and assistance with activities of daily living (ADLs) to participants in their homes, including bathing, dressing, grooming, toileting, and safe transfer.
• Assist participants with Instrumental ADLs, including laundry, light housekeeping, shopping, and light meal preparation.
• Measure and document participants’ vital signs as instructed.
• Act as a medical escort on vehicles to medical appointments, supporting safe transfer in and out of homes.
• Assist Rehabilitation team by carrying out home exercise programs as indicated in the care plan developed by the IDT, after sufficient training as needed.
• Provide translation services as needed.
