Job summary

WelbeHealth, LLC. is hiring Home Care Assistants to provide in-home personal care and support to seniors in our value-based program.

You will work full-time, visiting 2–5 participants daily, performing ADLs, and supporting care plans as part of the interdisciplinary team. You'll travel between participants with reimbursement for travel time and mileage, receive a competitive compensation package with base pay plus bonus, and enjoy medical/dental/vision coverage, PTO, holidays, a 401(k) match,

Qualifications

• High school diploma or equivalent.
• Current BLS/First Aid Certification required.
• CNA or HHA certification preferred.
• Bilingual preferred.
• Must be registered with Home Care Registry (HCA) prior to start date.
• CPR and First Aid certification within 180 days of hire; prefer certification at hire.

Responsibilities

• Provide hands-on personal care and assistance with ADLs in participants' homes.
• Assist with Instrumental ADLs such as laundry, housekeeping, shopping, and meals.
• Measure and document participants' vital signs as instructed.
• Serve as medical escort to appointments, assisting transfers in/out of home.
• Support Rehabilitation team by carrying out home exercise programs per care plan.
• Provide translation services as needed.
