Job summary

WelbeHealth is seeking Home Care Assistants to deliver personal care and support in participants' homes as part of the IDT, reporting to the Home Care Supervisor. You will visit 2-5 patients per day, provide ADL assistance, and help with daily activities within a full-time schedule.

Qualifications

• High school diploma or equivalent.
• Current BLS/First Aid certification required.
• CNA or HHA certification preferred.
• Must be registered with Home Care Registry (HCA).
• 1+ years caregiving experience preferred.

Responsibilities

• Provide hands-on personal care and assistance with ADLs at participants' homes.
• Assist participants with instrumental ADLs like laundry, housekeeping, shopping, and meal prep.
• Measure and document vital signs as instructed.
• Act as medical escort to appointments and support safe transfers.
• Support Rehabilitation team with home exercise programs per care plan.
• Provide translation services as needed.
