Job summary

WelbeHealth is seeking Home Care Assistants to provide hands-on personal care in participants’ homes as part of an interdisciplinary team. You will follow the care plan, support ADLs, and help with daily activities, while ensuring safety and comfort for aging participants.

The role offers a stable schedule with 8-hour days, travel between participants reimbursed, and inclusion in a comprehensive benefits package.

Qualifications

• High school diploma or equivalency.
• Current BLS/First Aid Certification.
• CNA or Home Health Aide certification preferred.
• Must be registered with Home Care Registry prior to start date.
• CPR and First Aid certification within 180 days of hire preferred.
• 1+ years of caregiving experience with elderly or senior patients preferred.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living (ADLs) to participants in their homes.
• Assist with Instrumental ADLs such as laundry, light housekeeping, shopping, and light meal preparation.
• Measure and document participants’ vital signs.
• Serve as medical escort to appointments and support safe transfers.
• Assist Rehabilitation team with home exercise programs per care plan.
• Provide translation services as needed.

Education

High school diploma or equivalency

BLS/First Aid Certification

CNA or HHA certification

Registered with Home Care Registry

CPR and First Aid within 180 days
