Job summary

WelbeHealth, LLC. is seeking a Home Care Assistant to deliver safe, in-home personal care and assist with daily activities for our participants. You will work with an interdisciplinary team to follow care plans and ensure participant comfort and safety in their homes.

The role emphasizes hands-on care, ADL support, vital signs documentation, and transportation assistance to medical appointments, with opportunities to contribute to care planning and translation services as needed.

Qualifications

• High school diploma or equivalency required.
• BLS/First Aid certification required; CPR certification within 180 days preferred.
• CNA or HHA certification preferred; HCA registration must be completed prior to start date.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living (ADLs) in participants' homes.
• Assist with Instrumental ADLs such as laundry, light housekeeping, shopping, and meal prep.
• Measure and document participants’ vital signs as instructed.
• Act as a medical escort to appointments and assist with safe transfers.
• Support Rehabilitation team by carrying out home exercise programs per the care plan.
• Provide translation services as needed.
