Job summary

WelbeHealth, LLC. is seeking Home Care Assistants to deliver safe personal care in participants' homes and join an interdisciplinary team. You will be assigned participants to fill a full 8-hour workday, visiting 2–5 patients daily, with travel time and mileage reimbursement.

The role emphasizes comprehensive patient care, ADL support, and collaboration with the Rehabilitation team. A stable full-time schedule is offered with competitive compensation including base pay and bonus, plus benefits.

Qualifications

• High school diploma or equivalency required.
• Current BLS/First Aid certification required.
• CNA or Home Health Aide certification preferred.
• Must be registered with Home Care Registry before start date.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living (ADLs) in participants' homes.
• Assist with Instrumental ADLs such as laundry, light housekeeping, shopping and meal prep.
• Measure and document participants' vital signs.
• Act as medical escort to appointments and assist safe transfers.
• Support Rehabilitation team by carrying out home exercise programs per care plan after training.
• Provide translation services as needed.
